El estiramiento de la pared del ventrículo izquierdo es el estímulo por el cual los miocitos de ese territorio del miocardio, desencadenen el proceso de sintesis del peptido natriurético B. La magnitud, el grado de estiramiento regula el nivel de producción. Por eso (BNP / NT proBNP) tienen un valor normal
En el interior del miocito, cuando se síntetiza ese péptido natriurético lo hace unido a una proteína dando origen a una molécula de alto peso molecular (Pre-ProBNP). Cuando la molécula es secretada a la sangre se escinde y hallamos en forma simultánea y separada el péptido natriurético (BNP) y la proteína con la cual estaba unida en el interior del miocito (NT-ProBNP). Por ende, como la relación entre las dos formas es equimolecular, cuando uno quiere analizar los niveles sanguíneos del péptido natriurético existen dos opciones equivalentes, medir BNP o la proteína con la cual estaba unido en el miocito NT-ProBNP.
Si bien la información clínica que se obtiene por ambas formas es equivalente, los valores son de magnitudes muy ditintas, las diferencias bioquímicas entre ambas moleculas generan esa disparidad en la magnitud de los valores informados, ello se basa fundamentalmente en vida media de ambas moleculas y el dinamismo de elimicación de cada molecula.
Los matices diferentes deben ser conocidos como así también aspectos que pueden llegar a afectar el valor medido, para una correcta interpretación de los resultado

Stretching of the left ventricular wall is the stimulus that triggers the synthesis of B-type natriuretic peptide (BNP) by the myocytes in that region of the myocardium. The magnitude, or degree, of the stretch regulates the level of production. This is why BNP and NT-proBNP have a normal range. Inside the myocyte, when this natriuretic peptide is synthesized, it is bound to a protein, giving rise to a high molecular weight molecule (pre-proBNP). When this molecule is secreted into the bloodstream, it is cleaved, and we find, simultaneously and separately, the natriuretic peptide (BNP) and the protein to which it was bound inside the myocyte (NT-proBNP). Therefore, since the ratio between the two forms is equimolecular, when analyzing blood levels of the natriuretic peptide, there are two equivalent options: measuring BNP or the protein to which it was bound in the myocyte, NT-proBNP.
Although the clinical information obtained through both methods is equivalent, the values are of very different magnitudes. The biochemical differences between the two molecules generate this disparity in the magnitude of the reported values. This is based primarily on the half-life of each molecule and the rate at which it is eliminated.
These different nuances, as well as aspects that can affect the measured value, must be understood for a correct interpretation of the results.
